VAR has a real problem with holding in the box, is it just them not seeing the incident or do they just not understand the rules?

I think it's a problem of unlike other fouls, holding usually isn't 100% the fault of one player. There's almost always a little bit of holding from both parties with one team usually doing more of it. That makes is really hard to pass the very high "clear and obvious bar".
Now I think the Kane hold and this one were both clear and obvious errors, but it seems the VARs have been instructed to have a very high bar when it comes to holding.
