Something that does cheese me off a bit and sometimes can really spoil extra time is when managers start planning for the penalties early and we have to sit through that extra sub, etc.

Why not just say subs can only be made during 90 mins but penalty takers can be selected from the whole matchday squad? That way teams can stop worrying about who's actually on the pitch and get on with playing.

Sure, you'll get some teams who will bench a specialist penalty taker maybe, but if they're no good in open play then that's a sacrificed spot so it would even itself out.
